The Massachusetts Department of Revenue has released 2022 Schedule HC: Health Care Information form, Schedule HC Instructions, Schedule HC-CS: Health Care Information Continuation Sheet. These state forms report insurance offerings to Massachusetts residents as required by state law. They are parallel to and independent of similar federal reporting requirements
The cap has been raised on annual deductibles of $2,750 for an individual and $5,500 for a family. For plans with up-front deductibles or co-insurance on core services, an annual maximum on out-of-pocket spending has been raised to no more than $8,700 for an individual and $17,400 for a family.
