Arizona has released its 2019 Publication 701, Optical Media Reporting. This publication contains information about reporting by optical media of Arizona withholding on federal Forms W-2, W-2c, W2-G and 1099-R (or any other Form 1099 equivalent), if reporting Arizona withholding.

There are some revisions to this publication of which filers should take note:

  • The publication now specifies that it applies to the reporting of Arizona withholding, not to all amounts paid and subject to reporting.
  • ADOR now accepts flash drives for optical media reporting.
  • In the RS-State Record, positions 298-307, provide the employer’s EIN as the State ID number; zero fill to the left. This change will not be required until tax year 2020.
  • Create the CD-ROM, DVD or flash drive using the format for Windows 7 (previously, Windows XP) or later operating system.
